PowerColor 3dfx Interactive, Inc. Voodoo Series Driver Driver. Note model name is Evilking 3 pro(166Mhz), model number of adaptor card however is not known at this time. I have used this driver myself and have had no problems in a Win 2K Pro environment, I haven't tried and can't verify whether TV out will function though.
